When you raise a request on behalf of a patient, you can star the questionnaires your practice uses most. Starred questionnaires always appear at the top of the list, so your team can open the right form straight away.

Click Create Request and find the patient as normal.

Click View full list to see every questionnaire available to your practice.

Find the questionnaire you want and click the star on the right-hand side of it.

Repeat for any other questionnaires your team uses regularly.

The next time you create a request, your starred questionnaires will appear at the top of the list, ready to select.

Tip: If you're looking for a more generic, medical pathway, we recommend starring General Assessment. It gives reception a fuller form to work through when a patient's request needs more detail than a quick query, and practices tell us it works well as their everyday proxy questionnaire.
