You can now build your own questionnaires in Anima for patients to complete. This is useful if your practice has a specific pathway you'd like to support, or if you need a questionnaire that Anima doesn't currently offer.
Where to find Custom Questionnaires
Go to Settings, select Organisation, then Questionnaires. This is where you create and manage all of your custom questionnaires.
Creating a questionnaire
Click New questionnaire.
Give your questionnaire a title, for example "Ask about my blood test results".
Choose whether requests should come in as an Admin or Medical request. This follows the same split used by Anima's standard questionnaires.
Add your first question and choose its type: single choice, multiple choice, free text, number, or date. For example, "When was your blood test?" with the type set to Date.
Choose whether the question is required or optional.
To add another question, click the box underneath your last question. For example, "Is there anything else you'd like to tell us?" as an optional free text question.
To reorder questions, use the arrows next to each question.
When you're happy with it, click Publish. Your questionnaire is now ready to use.
Making the questionnaire available to patients
Important: custom questionnaires are not automatically offered to patients as part of the standard admin or medical flows. Patients can only reach them through a link, which you can either add to your online consultation page, send to patients directly, or via Clinical Proxy Submissions (submissions made by your Reception team).
Adding the link to your patient page
In Questionnaires, find your custom questionnaire and click Copy link.
Go to Patient app and scroll to the bottom of the page.
Click Quick access links, then select Custom URL.
Paste in the link to your questionnaire and give it a name that patients will see, for example "Ask about a blood test".
Click to add the link.
The link will now appear on your patient-facing page.
Sending the link to patients
You can also send the questionnaire straight to patients. Click Copy link next to your custom questionnaire and paste it into a message to the patient, for example an SMS. This is handy when you want a specific patient or group of patients to complete it without having to find it on your patient page.
You may also wish to add the link to a Questionnaire to a messaging template via Settings > Organisation > Communication Template > SMS Message > +.
Submitting a custom questionnaire on behalf of a patient
Staff can also use custom questionnaires when submitting a request on a patient's behalf. When starting a request, click View the full list, and your custom questionnaires will appear alongside the standard ones. Select one to start it, then complete it as normal.
