For Staff: How to Create an Anima Account on Behalf of a Patient

You may need to create an Anima account on behalf of a patient, for example, if you need to create a proxy request for them.

Staff have two related but different tools:

A. Create a patient account

Open Create patient (via patient search/context when no Anima account exists). You can choose:

  • Standalone patient — full patient account at your practice

  • Dependant — patient linked to an existing caregiver account (caregiver must already be registered; search by their NHS number)

B. Submit a request on behalf of a patient (clinical proxy)

From the home grid, use Submit request on behalf of patient (proxy request modal):

  1. Search by the patient's NHS number

  2. If no Anima account exists → Create new patient account

  3. Complete the questionnaire on their behalf

Clinical proxy requests may be restricted by your practice's request limits (some practices allow staff to override).

Continuing to submit the form:

At the bottom of the form, in the Consent section, you will see:

“Please confirm the following with the patient before continuing”.

You must tick both checkboxes before you can click Create patient account:

  1. Contact confirmation Either:

    • The patient will be sent an email with their login details to the email address provided. They will need to log in to their Anima account on a computer or smartphone to access any follow up to their request.

or, if the patient has no email:

    • The patient will be contacted with the outcome of their request by telephone. Please ensure their registered phone number is correct and up to date.

  2. Record consent

The patient understands that any information they provide may be saved in their patient record.

These confirm the patient understands how their request will be handled and that you are submitting it on their behalf.

If the patient has no email, also tick This patient has stated they do not have an email address in the contact details section.

Click Create patient account.
