What's new when you send a request to your practice

We've updated the way you send a request to your practice. It's quicker, and we only ask you what's relevant to your problem.

Here's what to expect:

You still choose medical or admin

Nothing has changed at the start. You'll still be asked whether your request is a medical one or an admin one, such as a sick note or a query about a letter.

The safety questions come first

Before anything else, you'll see the standard NHS safety questions. If you tick anything here, we'll point you to the right urgent care straight away.

Tell us what's wrong in your own words

Instead of searching for the right form, you type a short description of your problem. For example, "I've had a sore throat and a dry cough for two days."

One problem at a time

If you mention more than one thing, we'll ask you to choose which to tell us about first. Once you've sent that request, we'll bring you back so you can send the others separately. This means each problem gets looked at properly rather than being missed.

We'll check we've understood

Based on your description, we'll pick the set of questions that fits best and show you what we think your request is about before you start. If we've got it wrong, you can go back and add more detail.

Shorter, more relevant questions

You'll usually answer around six or seven questions, and only ones that apply to you. Your age and other details are taken into account, so you won't be asked about things that aren't relevant.

You won't need to repeat yourself

If you've already mentioned something in your description, the related answers will be filled in for you. You just check they're right and carry on.
